The 1989 Exxon Valdez oil spill off Alaska, the Challenger space shuttle disaster and the Chernobyl nuclear accident have all been attributed to human errors in which sleep deprivation played a role.

Further most of the traffic accidents happen because the drivers are much sleep deprived that they lost control easily.

I remember an incident a few months back in which both the pilot and co pilot were asleep and therefor they flew past their destination. Thank God for automatic pilot.
